GECC Forms

Welcome to the Sherry and Ken Goldberg Family Early Childhood Center! Please complete each form below for your child (red form buttons are required forms).

All forms can be submitted online, with the exception of the ‘Health Information Card’ which needs to be downloaded, printed, and filled out by your child’s pediatrician. After completion, you may either fax (214-368-4709), click here to upload the PDF, or return to the GECC front office.

If you wish to purchase fall hot lunch or fall (or full year) challah, please contact the GECC front desk at 214-239-7117 or mederi@jccdallas.org.

Monday: “CHK’N” tenders, corn, and apple sauce.
Wednesday: Cheese Pizza, fresh fruit, and a cookie.
Friday: Pasta with butter, sauce on the side, vegetables, and apple sauce.
